What is a Patient Navigator?
A patient navigator is a skilled and compassionate healthcare professional who serves as your advocate and ally during your medical experience.
Trained in various aspects of healthcare, patient navigators work closely with patients, caregivers, and medical teams to ensure seamless coordination and personalized care.
WHAT DOES A PATIENT NAVIGATOR DO?- Emotional Support: Patient navigators understand the emotional toll a difficult diagnosis can have on patients and their loved ones. They offer a listening ear, compassion, and encouragement throughout the process.
- Information Guidance: Navigating the healthcare system can be complex, with numerous medical terminologies and treatment options to consider. Patient navigators provide clear and understandable explanations, ensuring that you are well-informed to make important decisions about your care.
- Care Coordination: Patient navigators collaborate with various healthcare providers, specialists, and support services to streamline your care. They schedule appointments, organize medical records, and ensure that all members of the healthcare team are on the same page.

Shea is a Nationally Board-Certified Physician Assistant with a heartfelt dedication to patient advocacy. Shea's journey as a medical professional has been marked by a commitment to providing exceptional patient care and holistic wellness.
Certified by the esteemed National Commission on Certification of Physician Assistants and proudly associated with the American Academy of Physician Assistants, Shea is also an active participant in the A4M Functional Medicine Fellowship, reflecting her pursuit of staying at the forefront of medical advancements and comprehensive patient care.
Driven by her passion for patient advocacy, Shea believes in providing comprehensive medical care that addresses not only physical ailments but also takes into account the patient's emotional well-being. Shea strives to deeply understand how her patients' lives are interconnected with their health concerns, enabling her to devise tailored treatment plans that truly encompass body, mind, and soul. Shea's unwavering commitment to her patients' well-being extends beyond treating illness.

Navjeet Cheema, FNP-BC
Navjeet is a Nationally Board Certified Nurse Practitioner with a passion for patient advocacy and expanding healthcare access. Through her nursing background, she has developed a profound commitment to providing care for individuals of all ages, skillfully combining medical expertise with compassionate attention.
Completing her master's degree in nursing and family nurse practitioner program at Samuel Merritt University and obtaining certification from the American Nurses Credentialing Center, Navjeet is well-equipped to navigate and guide patients through their healthcare journeys.
Her patient-centered approach revolves around conducting thorough diagnoses and treatments, striving to uncover the root causes of diseases and health concerns. Navjeet firmly believes in caring for her patients over the long term, acknowledging the importance of family context in making informed medical decisions.
In her practice, Navjeet places emphasis on the significance of food and lifestyle as integral components of health maintenance. Fluent in both English and Punjabi, she feels privileged to serve her community in Yuba City, her hometown.
